Today in healthcare, there is an increasing focus on dealing with the social determinants of health. These factors like housing, social support and transportation affect patient’s ability to access care, prevent health issues, and treat them when they do occur. In fact, studies show that only 15% of health outcomes are attributed to what goes on at the hospital or clinic. Providers need new tools to provide visibility into these factors ordinarily unseen by care providers. PARTNERme is our network-science informed solution to do just that. By interpreting PARTNERme social connectedness reports and adapting their care accordingly, providers can find new ways to increase support for patients and improve their health & well-being in the long run.
Interpreting PARTNERme Social Connectedness Reports to Improve Health and Well-Being
PARTNERme is our new tool for measuring and integrating social connectedness data into patient care coordination and treatment. Our relationships with family, friends, coworkers and others have a direct influence on our ability to connect with resources, access support and opportunities. This makes it a significant social determinant of health and well-being.
After patients complete a short, touch-screen questionnaire, PARTNERme creates a social connectedness report for their provider to review with them. It includes detailed, actionable information about the patient’s support network and the quality of connections, including how much they trust and depend on network members. They also receive social connectedness risk level and loneliness scores to flag individuals at risk and in need of more systemic interventions.
