Our First Template: The Default Community Partner Survey

Available now in your PARTNER account, this 18-question survey is our most popular and widely used instrument. It’s been deployed in over 1,000 communities across all 50 states and 20+ countries.

It helps you assess:

  • Partner Contributions – Funding, expertise, leadership, in-kind resources, and more.
  • Network Outcomes – From improving services to reducing disparities.
  • Collaboration Quality – Trust, value, mission alignment, reliability.
  • Connectivity & Relationships – Who’s connected, how, and why.

All of these measures feed directly into PARTNER’s analytics—so you can identify key players, visualize gaps, and make data-driven decisions to strengthen your network.

Additional Templates:

Early Childhood Network Survey

The Early Childhood Network Survey includes 25 questions created by our team through multiple projects mapping early childhood and maternal health networks across dozens of states.

The core of the survey is our 18-question validated community partner network survey. However, they are modified for an early childhood system context, with additional optional questions that are specific to this domain. This allows you to analyze the results in a similar way to our default survey template (see above).

We encourage you to edit the questions to fit your community and network context.

How to Use PARTNER CPRM Survey Templates

1. Go to Your Question Library in PARTNER CPRM.

2. Click the “Templates” button at the top (next to “Add” and “English”).

3. Use the dropdown menu to select your template (e.g., Default Community Partner Survey).

4. Review and Save – This will add the full question set to your library as a formatted Question Group.

5. When creating a survey, add the Question Group with one click—then customize as needed.

6. Launch your survey and start collecting data!
