Screening for Adverse Social Connectedness with the Mental Health Center of Denver

The Mental Health Center of Denver is Using PARTNERme to Identify Patient Need Gaps and Fill Them With Community Resources.

The MHCD 2Succeed education training programs are designed to provide the people we serve in our community with the education they need to lead successful, empowered lives. They used the PARTNERme screener and referral system as part of a pilot project funded by the Colorado Health Foundation.

Education counselors working with clients at the MHCD sent them a link to an individualized screening. Each client answered questions about their social needs, like housing and transportation, and drew a map of their existing social support. Education counselors used the information to tailor their support and provide community referrals to help. You can see a sample report similar to the one prepared for this project to the right.
