Mapping the North Central Accountable Community of Health Network to Strengthen Their Impact

The North Central Accountable Community of Health advances whole-person health and health equity in central Washington state by unifying stakeholders, supporting collaboration, and driving systemic change.

NCACH brings together clinical and community partners throughout our North Central Washington region in developing data-driven health priorities and programs to improve local health systems.  Their collective work helps support more equitable access to improving health and well-being for the residents in Chelan, Douglas, Okanogan, and Grant counties. They use collaborative partnerships and innovative solutions to improve the health of our communities as a part of the state’s Medicaid Transformation efforts.

As NCACH evaluates their progress and works to improve their impact, they want to know more about their network of community partners. These guiding questions include:

  • Who are our partners? Who are our partners’ partners?
  • Are we facilitating creation and maintenance of inter-organizational relationships?
  • Are we bridging or decreasing silos across sectors? Across geography?
  • Are we helping to build capacity that fills needs and gaps in the network?
  • Where are holes present in our network? 
  • Where should we be creating new connections and partnerships?

Visible Network Labs is supporting NCACH with ongoing evaluation support using the PARTNER CPRM. Our team will help them survey their partners to map and visualize their network, and lead them through a process of analysis, reflection, and discussion to create ongoing actionable insights for the future.
